Waiver and Release
In consideration of this entry being accepted, I hereby for myself, heirs, executors, and administrators, waive and release any claim I may have against The Brad Kaminsky Foundation, its officers and directors, the organizing committee, the sponsoring organizations,successors, or assignees for any injury that may be suffered by me at or in this event. I know that running, walking etc and participating in this event can be strenuous and cause exhaustion and is a potentially hazardous activity. I am medically able and properly trained for the dance marathon. I assume all risks associated with dancing in the event, including but not limited to, falls, contact with other participants, effects of weather including heat and humidity, exhaustion, sore muscles, mental changes and all such risks of participating in a dance marathon known and appreciated by me. Having read this waiver and knowing these facts and in consideration of acceptances of my entry, I, for myself, my heirs, executor, or anyone entitled to act on my behalf, waive and release The Brad Kaminsky Foundation and and all employees and volunteers from these entities, their representatives and succession from all claims of every kind or nature whatever, foreseen or unforeseen, known or unknown. I grant my permission to all of the foregoing to use any photograph, motion picture, video, or sound recordings, or any other record of this event for any legitimate purpose.
